How do I know what services I need?
A Physical Therapist or Registered Nurse will provide an initial evaluation to assess your care needs. This evaluation will help us in determining what services you may benefit from.
What insurances do you accept?
Achieve Rehab & Home Health is a Medicare/ Ohio Medicaid provider and also accepts many other payer sources. Please call (513)794-0555 to speak with a payment specialist for assistance with determining coverage. Our experienced staff will do a complimentary coverage evaluation and work directly with your insurance company or payer, whenever possible, to arrange for payment. Achieve handles the phone calls and the paperwork for you.
